A student wishing to file a petition must complete this form and provide a complete and concise statement of the problem and requested relief.

It is important that all pertinent facts be included. Completed forms will be sent to the Registrar's Office. To facilitate this request, please check the appropriate reason for the petition. Failure to follow these instructions will delay action on your request.

† Day students requesting waiver of 15 credit maximum. No student will be permitted to enroll in more than 16 credits in a semester.

†Evening student requesting waiver of 12 credit maximum. No student will be permitted to enroll in more than 12 credits if employed in more than 20 hours per week. Statement of employment hours must accompany petition.
